Age Well Neighbourhoods

Building on ongoing Age Well SG efforts, Age Well Neighbourhoods (AWNs) are being progressively introduced in towns with higher concentrations of seniors to bring enhanced care services and senior-friendly infrastructure closer to where they live.
AWNs complement the Government's ongoing efforts to expand housing-care models such as Community Care Apartments (CCAs) and Shared Stay-in Senior Caregiving Services, by providing more options for seniors who prefer to age in their own homes and within familiar neighbourhoods.
More accessible community touchpoints
Seniors can look forward to even easier access to active ageing programmes, as we will further expand the number of Active Ageing Centre (AAC) touchpoints in AWNs. This builds on our nationwide efforts through Age Well SG to better expand AAC coverage for our seniors.

Greater assurance for frail seniors who need more support
Enhanced Home Personal Care (HPC+) provides help with daily or personal care activities at home. Seniors in AWNs who are enrolled onto HPC+ will have access to expanded coverage and more timely support for unanticipated Activities of Daily Living (ADL) needs that arise between scheduled sessions or after-hours support. This expanded coverage is available at an additional fee. Please check in with your HPC+ provider for prevailing charges and subsides.

Community Health Posts (CHPs) are located within the neighbourhood to bring you and your loved ones convenient access to health services and social support, beyond doctor visits. Seniors in AWNs can access enhanced CHPs weekly. This includes a wider range of services such as frailty screening, regular health workshops, health coaching and telehealth consultations.

Improve our senior friendly infrastructure
We will continue to roll out senior-friendly infrastructure upgrading programmes nationwide, including at AWNs. These include the Home Improvement Programme (HIP) and Enhancement for Active Seniors (EASE) programme for HDB households, and the EASE (Private) programme for seniors residing in private estates. At the neighbourhood level, we will introduce senior-friendly features like fitness trails, barrier-free access, and dementia-friendly elements. Within our AWNs, we will also identify roads which are near key amenities to be transformed into Friendly Streets, for seniors to have safer, more comfortable, and convenient commutes within the neighbourhood.

Age Well Neighbourhoods will be progressively introduced in senior-dense towns
We will roll out Singapore’s first AWN at Toa Payoh in 2026, with further expansion to Bedok, Bukit Panjang, and Tiong Bahru-Redhill from 2027.
